Students may download the enclosed ICR form, fill-in the same and submit it with necessary fees and enclosures. ICR Form is not a replacement of CPT Prospectus. Students download this form will necessarily have to purchase CPE Prospectus and write the Application Number available in the CPT Prospectus on the download ICR Form.
